Pricing and Subscription Agreement.

Subscription Service Agreement

This Subscription Service Agreement (the “Agreement”) is between HLS Collective, LLC (“HLS”), and the participant (“I” or “me” or “Participant”). By purchasing, I acknowledge and agree to the terms of this Agreement.

PAYMENT TERMS:

In consideration of my participation in any exercise and/or coaching program through HLS Collective (“HLS”), I agree to an initial Three (3) Months Commitment (“Minimum Commitment Term”), although I am encouraged to continue beyond until goals are achieved. I understand that my subscription service purchase will renew automatically on a biweekly or monthly payment schedule, as I have designated below, unless paid-in-full. I understand and agree that cancellation of the subscription service beyond the Minimum Commitment Term must be expressed in writing via email to my HLS coach/personal trainer, with thirty (30) days advanced notice of the cancellation date. 

CANCELLATION TERMS:

1.    All personal training subscriptions must be completed consecutively.

2.    Personal training subscriptions are non-transferable.

3.    I will be charged the full subscription amount for the Minimum Commitment Term, even if I decide not to participate or adhere to my fitness plan within this period.

4.    Personal training sessions that are not rescheduled or canceled twenty-four (24) hours in advance will result in forfeiture of the session and will not be refunded.

5.    If I arrive late, I will receive the remaining scheduled session time, unless other arrangements have been previously made with the personal trainer/coach. 

My purchase of any HLS Service or Program indicates full compliance with the requests and the promises above, and complete understanding of the services to be provided.
